Short-Term Rehabilitation

The Center offers a comprehensive program of short-term rehabilitation to patients whose goal is to return home following hospitalization.

Complex medical needs our patients may present including:

• IV Therapy
• G-tube/J-tube Management
• Pain Management
• End-of-Life Care
• Ostomy Care and Instruction
• Diabetes Care and Instruction
• Complex Wound Care
• Care for Chronic Respiratory Illnesses

Our primary nursing model of care ensures residents will have consistent caregivers in order to foster close and trusting relationships that make a high quality of life possible in the nursing home setting.
