Admissions  

The majority of our patients enter the facility after a stay in an acute care setting.  All such patients are screened by the ANRC Admissions Committee to assure that the Center is an appropriate placement.

 

Families are encouraged to visit the Center and meet with the Executive Director, Director of Nursing, or Social Worker prior to admission; however, this step is not required for residents who are expected to return home after rehabilitation.  Hospital care managers and discharge planners should call admissions to make a referral. 

 

Some residents transition to our long-term care directly from home or an assisted living facility.  In such situations, a tour of the facility and a meeting with a member of the Admissions Committee is required in advance.  A financial application may also be required. 

 

Regardless of a resident’s path to entry, he/she can depend on the guidance and understanding of a team of professionals who are available to guide the resident and his/her family and to make the adjustment to a new home as pleasant and easy as possible.

  Patients who are unable to return home after rehabilitation may qualify to remain in the Center as a long-term care resident.  Thanks to the generosity of the Armenian Women's Welfare Association, the Center’s daily rates are typically lower than those charged by other facilities.  The Center also accepts MassHealth, and our staff is available to assist with making an application, if appropriate.  Upon request, the Center will provide referrals to elder care attorneys for assistance with estate planning.
